Jerry was born on May 18, 1965 in Mt. Pleasant, Utah to John Amos and Marlene (Madsen) McIntosh. He graduated from Manti High School in Manti, Utah in 1983. Jerry worked as a concrete finisher for all of his life. He was in the National Guard in Utah. Jerry enjoyed fishing, hunting and spending time with his son. Survivors include: the love of his life, Cindy Casey, of Checotah; one son, Bailey McIntosh, of Checotah; step daughter, Krissa Casey, of Checotah; his mother, Marlene Standlee, of Jerome, ID; sisters: Virginia Draper, of Checotah; twin sister, Sherry Horn and husband, Arnold, of Salina, UT; three brothers: Johnny McIntosh, of Salt Lake City, UT; Kendell McIntosh, of Gunnison, UT; Billy McIntosh and wife, Gail, of Centerfield, UT; nieces: Misty Keller, of Eufaula; Courtney Keller, of Checotah; nephew, Tyler Draper and wife, Amy, of Checotah; great nephews: Joshua and Justin Laughlin, of Eufaula; Jacob Draper, of Checotah; Vicente and Jose Fonseca, of TX; Daniel, John, Brady McIntosh, of UT; Clint Peterson, of UT; great nieces: Destiney and Brittany Draper, of Checotah; Claudia Fonseca, of Hanna; Holly, McKall, Meagan, and Mandy McIntosh; and Melissa Peterson, of UT; and a host of other family and friends. He is preceded in death by his grandmother, Bedelia Madsen; father, John Amos McIntosh; brother, Ronnie McIntosh; sister, Penny Foster; and sister in-law, Lynette McIntosh.
